The document is an application for a permit to export controlled substances from the U.S. to another country. The application is made under the Controlled Substances Import and Export Act and regulations prescribed thereunder. The applicant provides information such as consignee name, address, business, foreign port of entry, port of exportation, exporting carrier or vessel, approximate date of exportation, controlled substance name and quantity, DEA permit number, affidavit, exporter's information, and signature. The packages to be exported are labeled in accordance with regulations and the importing country has a system for controlling substances. The drugs are consigned to holders of required permits or licenses and will be applied exclusively to medical or scientific uses within the country. There is an actual need for controlled substances in the country, they will not be re-exported unless for bulk cocaine alkaloid, which may be processed within the country and its products can be re-exported.
