This document outlines instructions for reporting a person to Norwegian authorities or a foreign embassy. Required documents include those listed on UDI's checklist available at www.udi.no/sjekklister. The form must be completely filled out and easily readable. If the report lacks necessary information or documents, it will be denied. Reports must be submitted before the person turns 18 years old. If desired, names can be written differently than registered or changed along with the report. For each individual: date of birth, place and country of birth, last name, first name(s), previous name(s) if applicable, reporting on Norwegian citizenship for adopted children and those with a Norwegian father, application location and date, stamp from police district or foreign embassy, DUF-number, gender (male or female), contact information including address, postal code, country, telephone number, and email. UDI will use contact details to communicate during future applications. If the mailing address for the response is not the one registered in the population registry, provide the alternate address. For those applying on behalf of a child: child's name, date of birth, place and country of birth, citizenship status, gender, and contact information. The child must give consent if over 12 years old. The application location, date, and all signatures are also required.
