This form should be used by Federal Firearms Licensees who require additional space while filling out ATF Form 3310.11, Section C - Description of Firearms.

This document is a continuation sheet for reporting the theft or loss of firearms from a Federal Firearms Licensee's inventory or collection to the U.S. Department of Justice, Bureau of Alcohol, Tobacco, Firearms and Explosives. The report requires information about each firearm including manufacturer, importer, model, serial number, type, caliber/gauge, acquisition date, and certification from the licensee. Failure to report within 48 hours is a violation of federal law.
