Form 13560 is a tax form from the IRS for Health Plan Administrators to return the Government's portion of insurance premiums. The form must accompany all returned funds. HPA returns may be made via check, EFT reversal or uncashed US Treasury check. Detailed information including insured name, SSN, date coverage ended, reason for termination, total amount returned and reason for returned funds must be provided for each impacted individual. The form also includes instructions regarding Paperwork Reduction Act and Privacy Act.
