This document provides instructions for claimants who have been notified of a whistleblower award under Section 7623(b) and wish to apply for a reduced rate of withholding tax. The application form, Form 14693, must be submitted within 30 days from the date printed on the form. The form requires claimant and spouse information, address details, identification number, claim numbers, attorney information, attorney fees and court costs, and supporting documents.
