Form 2678 (Rev. 12-2023) is an IRS document for employer/payer appointment or revocation of agent for tax reporting, depositing, and payment of employment or other withholding taxes. The form includes two parts: Part 1 for Employer or Payer Information which requires checking one box for appointment or revocation, filling in EIN, name, address, forms for which the appointment is granted or revoked, and signing; and Part 2 for Agent Information which requires an agent's EIN, name, address, signature, and date. The form becomes effective only upon IRS approval. The document also includes instructions on disclosing confidential tax information to the agent, third-party contractors, and penalties of perjury.
