Form 4137 is used to report tips and allocated tips that were not reported to your employer. Report all cash and charge tips you received during the year, including any unreported or late reported tips and any tips you weren't required to report due to them being less than $20 in a calendar month. Tips subject to social security and Medicare tax should be reported in column (d) of Form 4137, while tips not subject to these taxes should be reported in column (c). If you had more than five employers, complete additional Forms 4137 and attach them to your primary form. The total tips reported in columns (c) and (d) should be combined in lines 2 and 3, respectively. Report any railroad retirement compensation on line 8, but make sure it doesn't exceed the $160,200 limit for the 6.2% rate. Subtract the amount of tips subject to only the 1.45% Medicare tax from the total tips reported if necessary for comparing lines 6 and 9.
