Form 4361 is used by ministers, members of religious orders, and Christian Science practitioners to apply for an exemption from self-employment tax. The form must be filed along with supporting documentation such as proof of ordination, commissioning, or licensing, and a letter from the governing body of the church or religious order stating that it is exempt from federal income tax under section 501(a) as a religious organization described in section 501(c)(3). The application must be submitted within 90 days of receiving the statement from the IRS confirming the grounds for exemption. Once approved, the exemption cannot be revoked.
