Form 4768 is a request for extension of time to file and/or pay U.S. estate (and generation-skipping transfer) taxes. Part I requires identification of the decedent, executor, and estate details. Part II requests an extension of time to file Form 706, 706-A, 706-NA, or 706-QDT. Extension for cause and additional extensions are also available. Part III requests an extension of time to pay taxes. The form must be signed by the executor or authorized filer and mailed to Internal Revenue Service.
