Form 656-PPV (Rev. 4-2020) is related to the Offer in Compromise process at the Internal Revenue Service (IRS), Department of the Treasury. If a Periodic Payment Offer is made, individuals are required to continue making payments during the investigation phase until a decision letter is received. Payments should be made payable to ‘United States Treasury’ and sent with a voucher that includes the taxpayer's name, address, social security number or employer identification number, offer in compromise number, and payment amount. Payments can also be made electronically through the Electronic Federal Tax Payment System (EFTPS). Taxpayers who qualified for Low-Income Certification are exempt from making payments. The voucher and payment should be mailed to either Memphis or Brookhaven IRS Center depending on the taxpayer's residence.
