[Form 8941] This form is used to claim the credit for small employer health insurance premiums on tax returns. Employers who provided health insurance coverage through a Small Business Health Options Program (SHOP) Marketplace, or are exempt from this requirement, should follow the instructions to determine if they are eligible to file Form 8941. Employers filling out the form need to provide information on the number of individuals employed, their average annual wages, and premiums paid for health insurance coverage. The form calculates the credit based on a percentage of these amounts. Employers should refer to Worksheets 1 through 7 for detailed instructions. The final amount is reported either on Form 3800 (for partnerships, S corporations, cooperatives, estates, and trusts), Form 990-T (for tax-exempt small employers), or Schedule K (for partnerships and S corporations).
