Form 941-X is used to correct employment tax returns (Form 941 or Form 941-SS). The process you use depends on the type and timing of errors. Underreported tax amounts should be corrected using the adjustment process, while overreported tax amounts can be corrected up to 90 days before the period of limitations expire by choosing either the adjustment process or the claim process. If filing within 90 days of the period of limitations expiring, both processes must be used.
