Resident Visa Declaration Form

Use this form if you have authorised another person to submit an online application for a resident visa on your behalf, including if you are a child aged over 18.

The Resident Visa Declaration Form for December 2021 is used by applicants who wish to apply for a resident visa and require someone else to submit the online application on their behalf. The form requires the applicant to agree to certain statements regarding their application and any visa granted, as well as declare that there are no outstanding matters or warrants that could affect the assessment of their good character. The form also authorizes Immigration New Zealand to make necessary enquiries about information provided on the form and to share this information with other government agencies to the extent necessary to make decisions about the applicant's immigration status. Additionally, the form requires the signature of the principal applicant, any accompanying dependent children over 18 years of age, and any person who will submit the online application on their behalf.
