This document is an application form for amending a Sri Lankan travel document. The applicant is required to fill in their present travel document number, surname, other names, permanent address, and specify the desired amendments. For instance, if there is a need to change the surname or add names after marriage, the certificate of birth/marriage proof should be submitted. Similarly, for professions/designations or job descriptions, relevant documents must be forwarded. If an identity card number is missing or changed, it should be written and submitted accordingly. The validity period can be extended for up to five years, with a fee of Rs. 200.00 per year. A passport with a restricted endorsement requires submission of supporting documents for amendment/deletion. Validation fees vary depending on the destination: Rs. 5,000.00 for all countries and middle-east. Fees are subject to revision.
