Driver License Record Correction Request (DL 207)

Use this form to dispute an incorrect traffic violation or conviction information on your driving record, other than motor vehicle traffic accidents. You must submit certified court abstracts an/or court documentation in order for the DMV to update your record.

The document outlines the procedure for correcting traffic violation or conviction information on a driver record with the Department of Motor Vehicles (DMV) in California. The form, DL-208 "Traffic Accident Record Correction Request," must be used for this purpose. DMV requires supporting documents from the court reporting the violation or conviction, such as a Court Abstract/Document Error form (DL 157) or an original signed or certified letter from the court on official letterhead. Traffic violation and conviction information cannot be corrected without these documents. The form, along with accompanying documents, should be sent to the DMV's Mandatory Actions Unit, and it may take four to six weeks for a response. Penal Code Section 115(a) states that falsifying or offering false instruments for filing or recording is a felony.
