Request for Driver License/Identification Card Status and Record Information

This form is used by commercial requesters, both end users and vendors, to request driver license (DL) record printouts, current copy of DL 44, etc. In order to use this form, a business entity must have a requester code with the department. This form is also used by our Employer Pull Notice (EPN) customers to obtain DL record information on prospective or casual drivers.

This document outlines instructions for requesting driver license and identification record information from the Department of Motor Vehicles (DMV). There are two methods to submit a request: by driver license or ID number and subject’s full name, or by subject’s full name and birth date. Requested information includes status and record data, order of suspension/revocation, guarantor signature search (DL 44), current copy DL 44, certified status, and comments. Additional requirements include the requester's code, user agreement number, driver license/ID number (required for Electronic Privacy Notice), name, birth date, total number of requests, and requester’s driver license/ID number (required). The document also specifies that no fee is required for certified status and provides contact information for the DMV Information Release Unit.
