Gender Self-Designation Form (Spanish - Español)

The District of Columbia's Department of Motor Vehicles (DMV) provides a form for individuals to change or determine their gender designation on a driver's license or ID card. To do so, applicants must: present a completed and signed gender designation form, pay the corresponding fees when requesting a new or modified license, and submit a recent photograph. Applicants should indicate the gender designation that best fits them, as transgender individuals often identify as male, female, or neither of the two, with the option for X. The DMV is committed to processing these requests respectfully and privately. Solicitors are not required to have changed their gender designation in other documents, nor must all forms of identification match. Name changes on licenses and ID cards can be processed by submitting the necessary documentation, and name changes also reflect on the Social Security register. Gender designation changes can be made at any time, regardless of previous designations. Applicants must present a recent photograph that complies with DMV's photo requirements and provide their full name, current address, social security number, license or ID card number, and signature.
