Parental Consent Form (Korean - 한국어)

DC government requests parents or legal guardians to visit dmv.dc.gov or call 311 (inside DC) or 202-737-4404 (outside DC) for inquiries. For reporting tip about DC government employees' misuse, waste, fraud or abuse, call DC Inspector General at 1-800-521-1639. To obtain a consent form for obtaining a learner's permit, applicants need to provide their performance name, date of birth, gender, social security number (optional), and DC address with city and zip code. Applicants should also indicate their relationship to the parent or legal guardian: parent or legal guardian. If legal guardians provide false information or intentionally make false statements on the form, they may face a $1,000 fine and/or 180 days in jail (D.C. official law §22-2405). Applicants need to submit one of the following documents depending on their relationship: parents - a complete birth certificate or court order; legal guardians - a court judgment number, parent's name, date of birth, gender, phone number, and DC address with city and zip code. All applicants must provide their driver's license or ID card expiration date and email address.
