This declaration is required when a minor cannot independently process an identification card request. The responsible adult, be it a father, guardian, or another responsible person, must provide acceptable identification documents for the minor according to the requirements listed in the 'Acceptable Identification Documents' chart for issuing a state identification card. The declaration should only establish and verify the relationship between the minor and the responsible adult. It cannot be used for any other purpose. The responsible adult must identify themselves by: accompanying the minor applicant to the office, signing the Identity Declaration form in the presence of an employee, presenting valid identification such as a valid Illinois driver's license or state identification card, or meeting other acceptable identification requirements. The form must be signed in the presence of an employee at the Secretary of State office. The document includes blanks for the responsible adult's name, relationship to the applicant, and their address and identification number.
