This document outlines the process for voluntarily surrendering a Kentucky driver license. Reasons for surrender include insurance purposes, no longer wanting to drive, and health reasons. Surrenderors must provide their name, date of birth, address, Kentucky license number, class license being surrendered, and any endorsements or restrictions. If the individual decides to reapply for driving privileges, they must return to a KYTC Driver Licensing Regional Office. Those who surrender a license have up to five years from their last valid license without needing to retest, unless their driving privileges were suspended. This does not apply to permit holders.
