Non-U.S. Citizen Application - Spanish

This document is an application form for non-U.S. citizens residing in Kentucky who wish to obtain a driver's license, permit, or ID. Applicants are required to fill out this form and submit it along with specific documents to the Division of Driver Licensing or a district office. The following information is needed: name, phone number, date of birth, sex, weight, height, eye color, Social Security Number (or proof of ineligibility), address, city, state, zip code, country of citizenship, and type and expiration date of the USCIS document(s). Applicants must also present one valid original document from a list: visa with I-94, work permit, I-94 issued by USCIS, or passport with entry stamp. In addition, applicants must provide certain documentation: Social Security card or proof of ineligibility, proof of residence (address verification within the last year), DS2019 for exchange visitors, recent employment verification for L1, L2, H1B, H4 individuals, and F1 student's I-20 and proof of enrollment. Finally, applicants must sign under penalty of perjury.
