This document is a revised individual firearm permit application form (IFP-FORM-01) from May 17, 2021. It outlines procedures for determining if applicants who are not U.S. citizens are prohibited from possessing firearms under Federal law based on their citizenship status and whether they fall into any exemptions. Applicants need to provide their full name, place of birth, country of citizenship, INS-issued alien or admission number, and answer questions related to their illegal presence in the U.S. and non-immigrant status. Non-immigrant aliens must determine if they are exempt from the prohibition under 18 U.S.C. 922(y) by answering questions 6a and 6b. Possible exemptions include having a valid hunting license or permit, being an official representative of a foreign government, or receiving a waiver from the Attorney General.
