CDL Self-Certification Classification and Medical Documentation

This form allows commercial drivers to change their certification status and submit a Medical Examiner's Certificate (MEC), if required for their type of operation.

This document is a self-certification form for commercial drivers in Montana, allowing them to declare their status as Interstate Excepted or non-excepted. The form requires the driver's identifying information, date of birth, driver license number, daytime phone number, and address on driver license. Drivers must certify that their commercial driving activities fall under one or more categories of excepted interstate commerce, such as federal or state government employment, transportation of human corpses or sick persons, fire truck or rescue vehicle drivers during emergencies, propane winter heating fuel delivery in response to an emergency condition, custom harvesting, beekeeping, private motor carrier of passengers for non-business purposes, or transporting migrant workers. Drivers who only operate in excepted interstate commerce do not need a federal Medical Examiner's Certificate. Those who do not meet this requirement must carry and provide a current MEC while operating in a non-excepted interstate commerce status.
