Request for NYS Driving Privileges

Used by out-of-state drivers to request restoration of their New York State driving privileges following a revocation of privileges. This form cannot be used to request restoration of a NYS driver license or learner permit following a revocation. Revoked NYS drivers must apply for a new license using form MV-44.

This document outlines the procedure for requesting New York State driving privileges. The required information includes a full name, date of birth, sex, daytime phone number (optional), current out-of-state residence address, mailing address (if different), and signature. Additionally, there are two fee structures depending on the processing date: $25 for requests processed before August 18, 2017, and $100 for requests processed on or after that date. Applicants must pay the appropriate fee using a check or money order made payable to ‘Commissioner of Motor Vehicles’ and send the form and payment to NYS Department of Motor Vehicles in Albany.
