Quit Driving Form

[Form 735-7211: Surrender of Oregon Driving Privileges and Application for Free ID Card] Individuals who recognize their inability to operate motor vehicles, including motorcycles and mopeds, can surrender their Oregon driving privileges using form 735-7206. Those eligible for a free ID card under the At-risk program should check a box and meet certain qualifications. Qualifications include but are not limited to paying for and passing all tests, submitting a Driver Medical Report, and surrendering the driver license or permit. Upon submission of the form, individuals will receive an identification card valid until the expiration date of their current license or permit with the same picture. Eligibility for a free ID card requires the Oregon driver license or permit to not be expired and the photo to be less than 9 years old. Individuals who do not meet these qualifications but wish to obtain an ID card or surrender an Oregon non-Real ID driver license for a REAL ID Identification card can visit a DMV office or go to DMV2U.oregon.gov to make an appointment. Forms should be mailed or faxed as directed.
