[DL-64 06/2022] - Instructions for completing a change of address or replacement application for Texas Driver License (DL) and Identification Card (ID). - Applicants must provide personal information including citizenship status, driver's license number, identification card number, date of birth, phone number, social security number, suffix, last name, first name, middle name/birth surname, residence address, city, county, state zip code, email, and mailing address. - Applicants can choose to add a veteran or disabled veteran designator on their DL or ID along with proof of honorable discharge and disability documentation. - Applicants may opt to support various programs including the Glenda Dawson Donate Life Texas Donor registry, organ donation, survivors of sexual assault, and DL/ID for foster or homeless youth. - The form includes certifications regarding residency, felony convictions, mental incapacitation, voter registration, and agreement to report address changes. - Applicants are reminded not to give false information and that perjury is a crime under state and federal law.
