Vermont Residency Certification - Mandarin Chinese

To use as proof of Vermont residency for applicants who are minors or applicants who are over the age of eighteen (18) and who reside with others.

To apply for a Vermont driver's license or non-driver ID, you need to provide proof of Vermont residency. If you don't have two pieces of mail with your name and Vermont address, you must submit one of the following documents along with the Vermont Residency Affidavit (VL-002 form): water or electric utility bill, property tax statement, Vermont Sales Tax Certificate of Registration (LC-142), EBT or AIM ID card, rental/homeowner insurance policy, etc. If you live with someone who doesn't receive mail at the Vermont street address: fill out the Vermont Residency Affidavit for Vermont residents (VL-002 form) and submit a proof of residency document from the person sharing the household. Military personnel stationed in Vermont follow the same rules as Vermont residents. If you live outside Vermont but are a parent or guardian of someone applying: fill out the Vermont Residency Affidavit for Vermont residents (VL-002 form) and submit one proof of residency document and a proof of relationship to the applicant.
