Driver Improvement Clinic Classroom Instruction Attendance Roster

Use this form to document instructor and student attendance.

This form is used by driver improvement clinics (DIC) to document instructor and student attendance. The DIC must conduct 8 full hours of instruction prior to issuing a certificate of completion and awarding safe driving points. The form requires the clinic name, code, date, instructor's name and signature, site code and address, clinic completion date, beginning and ending times, day of the week, customer's full legal name and DMV number, reason code for attendance, whether or not the student should be awarded safe driving points, total number of students, and total amount of fees submitted. The instructor must sign the certification statement, issue a certificate of completion to each student, calculate and submit a $10 processing fee for each student, and mail the completed roster with payment to the Virginia Department of Motor Vehicles within 5 days of course completion.
