Entry Level Driver Training Self-certification Of Exemption

Use this form if you held a commercial driver's license (CDL) prior to February 7, 2022 and are exempt from the federal Entry Level Driver Training requirements under 49 CFR parts 308, 383 and 384 and Va. Code § 46.2-341.12.

This form is for individuals who held a commercial driver's license (CDL) prior to February 7, 2022 and are exempt from federal EntryLevel Driver Training requirements. To qualify for the exemption, the applicant must have held the classification and/or endorsement(s) before the mentioned date. The form requires the applicant to provide their full legal name, commercial driver's license number, address, birthdate, daytime telephone number, email address, prior commercial driver's license history including issuing state or jurisdiction, license number, issue date, and CDL class and endorsements. The applicant must certify that all information provided is true and accurate under penalty of perjury.
