CDL Entry Level Driver Training (ELDT) Re-examination Course Completion Self-certification

This form may be used by commercial driver's license (CDL) applicants who failed the CDL general knowledge or road skills test three times prior to February 7, 2022 to self-certify to the completion of Entry Level Driver Training in order to satisfy the requirement to complete driver training prior to testing a fourth time.
