Va Community College System Confirmation of Students Issued a Commercial Driver's License

Use this form to request confirmation that students who successfully completed a CDL training course and examination administered by a Virginia community college that participates in DMV’s Third Party Tester Program were issued a CDL license by DMV.

This document is a request form for the Virginia Community College System (VCCS) or driver training schools (DTS) to confirm that students were issued a Commercial Driver's License (CDL) by the DMV. The form must be completed and signed by an authorized representative of the college or DTS. Each student listed on the form must have signed a Student Authorization for Release of Information form before submission.
