This company, as an employer or its agent, grants permission for disclosing driving records of employees, prospective employees, or volunteers upon receiving their authorization using this form. The form should be returned to the company after completion by the individuals. For record keeping purposes, completed forms must be kept for at least five years. If a prospective employee or volunteer was not hired, the consent form should be destroyed within 30 days. A new consent form is required if a driver returns to work after an absence. Driving records may only be used for the stated purpose and not disclosed to third parties without legal authorization. Individuals can provide a copy of their court order regarding criminal records. The company section should be filled in by the company or its agent. The form should then be provided to the employees, prospective employees, or volunteers. In the event of damages arising from disputes related to the disclosure of driving records, the Washington State Licensing Department is indemnified. The signatories affirm that the information provided is true and correct, with penalties for false statements under Washington law.
