Request for DUI Hearing | Washington (Russian)

The Department of Licensing (DOL) is planning to suspend, revoke, annul or revoke your driving permit, learner's permit or right to operate a vehicle. You have the right to request a formal hearing to contest the suspension or revocation of your driving privileges. The hearing will be conducted in accordance with Part 308-101 WAC. For information on the issues addressed at the hearing, see RCW 46.20.308. If you do not meet the requirements of the financial assistance program (see requests for financial aid below), you must pay a non-refundable fee of $375 with your application. The postmark on the application envelope should be dated no later than 7 days from the date of arrest or within 7 days of receiving the notification. If your request is not submitted within this time frame, you will lose the right to a hearing. Send the completed form along with a check or money order to Department of Licensing, Hearings and Interviews, PO Box 9048, Olympia, WA 98507-9048. Requests for financial assistance: If you are filing for financial aid (see definition in RCW 10.101.010) and wish to be exempted from the hearing fee, complete the Financial Assistance Application, which is available on our website: dol.wa.gov. All correspondence will be mailed to the address indicated in your file at DOL. Update your address online on the site dol.wa.gov or in person at your local driver licensing office.To receive an invitation to the hearing, please write to us with your full name, date of birth, 10-digit phone number, driver license number, state, date of arrest, name of attorney (if applicable), address of attorney (house number and street or PO box, city, state, zip code), 10-digit phone number of attorney, 10-digit fax number of attorney, and email address of attorney. If any party or witnesses do not speak English or have hearing impairments, a qualified interpreter will be provided free of charge.
