Request for DUI Hearing | Washington (Spanish)

This document is a notification from the Department of Licensing (DOL) regarding an intent to suspend, revoke, or deny a driver's license due to DUI charges. The recipient has the right to request a formal hearing to challenge the suspension or revocation. The hearing will be conducted in accordance with Chapter 308-101 of the WAC and Section 46.20.308 of the RCW. The request must include a non-refundable fee of $375, unless financial assistance is applicable. The form and payment should be sent to Hearings and Interviews at the Department of Licensing. Those with a valid Washington state driver's license and MasterCard, Visa, or American Express credit card may apply online. The deadline for filing the application is within 7 days of the arrest or within 7 days of receiving the notice. Failure to file within this period results in waiver of hearing rights. If financial assistance is sought, use the Financial Assistance Application available on the website. All correspondence will be sent to the registered address with the DOL. Information required includes full name, date of birth, phone number, email for invitation, driver's license number, state, arrest date, attorney name and contact details, language and dialect for interpreter if needed, and validity of current licenses.
