Report of Withholdings and Contributions for Health Benefits By Enrollment Code

This document is a revised version of Standard Form 2812-A, used for reporting withholdings and contributions for health benefits by enrollment code. It is issued by the Office of Personnel Management (OPM) and is applicable to CSRS/FERS personnel and payroll offices. The form includes fields for report number, bureau or office name, pay period dates, address, date payroll was paid, agency telephone number, enrollment codes, total withholdings and contributions for each enrollee, and the number of enrollees. This form must be completed for the last payroll periods paid during the 1st through the 15th of March and September.
