[Form N-426 Edition 03/09/23] This document is a request for certification of military or naval service for the U.S. Citizenship and Immigration Services (USCIS). Part 1 contains information about the service member, including their A-Number, USCIS Online Account Number, date of birth, full legal name, and branch of service. The service member certifies the release of their information for USCIS to determine eligibility for an immigration benefit. Part 2 is the service member's certification and signature authorizing the release of information. Part 3 is the certifying official's attestation of the service member's honorable service, including their full name, rank, title, and current branch of service since the start date. Part 4 includes any derogatory information regarding the service member's character, loyalty to the United States, criminal or unlawful conduct investigations, pending court martials, convictions, discharges, and fitness for citizenship.
Form N-426 is used to certify a service member's military or naval service for naturalization purposes under sections 328 and 329 of the Immigration and Nationality Act. The form must be completed by both the service member applying for naturalization and the certifying official from the executive department in which they are serving.
After USCIS accepts the certification, they will check it for completeness. If completed and certified by both you and a certifying official, it serves as an authenticated record of military service.
