Salvage Vehicle Notice of Retention By Owner

This document is a Notice of Retention by Owner for a salvage motorcycle. The form requires the vehicle identification number, engine number, make, California license plate, owner information including name, address, city, state, and zip code, insurance company details, date authorized signature, print name, insurance company name and address, date of loss claim number, daytime telephone number, and a mail completion form for the Department of Motor Vehicles. The vehicle owner must surrender the Certificate of Title and license plates within 10 days of settlement of loss and apply for a Salvage Certificate. The Department of Motor Vehicles will reflect a "Salvaged" notation in its database record for the vehicle.
