Gross Vehicle Weight Declaration

This document is a form for declaring Gross Vehicle Weight (GVW) for Special Mobile Machinery, Trucks, Tractors, or Truck Tractors with an empty or combined weight exceeding 16,000 lbs. Proof of Heavy Vehicle UseTax payment or exemption is required for vehicles with a GVW of 55,000 lbs. or more unless purchased within 60 days of registration. The form is also used to declare carrier type and total miles traveled for the previous twelve months for vehicles registered with Gross Vehicle Weight (GVW/TVW) license plates and Special Mobile Machinery operated over public highways more than 2,500 miles. Required information includes Vehicle Identification Number (VIN), Year, Make, Model or Body Style, Color, Agent/Owner daytime phone number, and calculation of Gross Vehicle Weight using the equation: Empty weight of the vehicle + empty weight of the trailer (if applicable) + combined empty weight of vehicle and trailer (if applicable) = heaviest load carried + declared gross vehicle weight (GVW) = total miles traveled. The form includes certifications for individual or company use only, and common/contract vehicle usage.
