Kentucky Transportation Cabinet's Division of Motor Vehicle Licensing form for a Kentucky Certificate of Title or Registration, referred to as TC 96-18201/2023. Applicant fills in the type of application desired: Duplicate Title or Transfer First Time Salvage Classic. If Duplicate is checked, original Certificate of Title number must be provided. Vehicle identification section includes VIN, year, body style, model, model no., color, motor no., cylinders, and truck weight (if motorcycle). Certified Inspector Section: Certified Inspector prints name and phone number, certifies the vehicle's roadworthiness and consistency of supporting documents. Title Brand Disclosure: Check appropriate block for Rebuilt Vehicle or Water Damage, with jurisdiction and title number if previous brand was issued. Odometer Disclosure with caution statement and mileage certification. Total Consideration Information: Sale Price, Trade In, Net Cost, Tax, Date of Sale, and certifications by seller and buyer. Joint Ownership: If neither box is checked, Title Transfer requires both signatures. Personal information: Name, Driver's License number and state, Email Address, Loan application statement (with or without application), Lessee name or other first lienholder, Holder address, County, State, Zip code, Lien to be filed in, Seller signature, Owner/Buyer signatures, Date of transfer, and Attesting Official Title. Sworn statements by seller and buyer regarding true and correct total consideration information.
