Affidavit for Cancellation of Registration

Complete this form to cancel a registration when there are two owners of a vehicle listed on the title/registration OR to cancel a registration on behalf of the owner(s).

This document is an affidavit for canceling vehicle registration. Sections A, B, and C are for the owner's information, vehicle details, and non-owner cancellation info respectively. Sections A, B, and D apply to owners and non-owners accordingly. Acceptable identification is required at the time of cancellation. The form is used when there are multiple owners or someone is canceling on their behalf. It can also be done online via Mass.Gov/RMV.
