This document is for those converting bicycles to motorized bicycles (mopeds) in Massachusetts. The affidavit should be completed and attached to a registration application. Bicycles with two or three wheels, an automatic transmission, a cylinder capacity of 50 CCs or less, and a maximum speed of 30 MPH or less are classified as Motorized Bicycles. If the bicycle does not meet these requirements, it cannot be registered or legally operated on public ways with the motor running. A Low-Speed Electric Bicycle, defined by Federal law as having two or three wheels, pedals, an electric motor under 750 watts, and a maximum speed of less than 20 MPH, does not need RMV registration. The form requires the owner's information, vehicle information including date of purchase, serial number, brand name, modification details, and signature.
