Junking or Scrapping a Motor Vehicle

Complete this affidavit if you have junked or scrapped a vehicle.

This document is a form for junking or scrapping a motor vehicle, according to Massachusetts General Laws (M.G.L.) Chapter 90D, Section 20(e). It requires the person junking the vehicle to provide their information, including last name, first name, middle initial, suffix, business name (if applicable), address, email address, and phone number. The vehicle's information, such as model year, make, VIN #, title # (registration # if exempt), state, and vehicle purchaser or receiver's information with their name, address, date of purchase or receipt, are also required. The document must be mailed to the Title Division with the certificate of title. A certification and signature from the junking company is required, attesting that the motor vehicle was junked or scraped on a specific date.
