Application for Disabled Person Placard

A Missouri disabled person placard is a removable windshield placard issued to physically disabled persons, parents or guardians of physically disabled persons, and not-for-profit groups or organizations that transport physically disabled persons. The placard must be hung from the front, middle rearview mirror of a parked vehicle in order to park in designated disabled parking spaces. It is unlawful to hang the placard from the rearview mirror when driving the vehicle. To apply for a new or renewal permanent or temporary placard, submit a completed Form 2769 and a Physician's Statement for Disabled Person's Placard (Form 1776) along with the required fee to the Department of Revenue. There are different requirements for permanent and temporary placards. Penalties for misuse or fraudulent application, renewal, procurement, or use of a placard include fines and a class A misdemeanor charge.
