Leasing/Rental authorization: new car inspection stickers

This document is an authorization form for ordering, security, and distribution of new car inspection stickers from the New Jersey Motor Vehicle Commission. Authorized signatories must sign the order form, providing their name, title, address, and home number. Orders can be placed by mail or fax.
