Application For Nursing Home Resident Parking Placard

This document is an application for a Nursing Home Resident Parking Placard issued by the New Jersey Motor Vehicle Commission (NJMVC). The placard is limited to nursing home owners or operators for use in vehicles owned or operated by the nursing home when transporting residents with disabilities.
