This document is a certification and application form for obtaining a certificate of title and registration of a motor vehicle with the North Dakota Department of Transportation. The form includes various scenarios for acquiring a vehicle such as gift, joint tenancy, inheritance, change of name, lease purchase agreement, dealer resale, disabled American veteran or former prisoner of war, non-profit senior citizens' corporation, mobility impaired person purchasing specially equipped vehicle, homemade vehicles, newly formed partnership corporation, transfer into family trust, military home of record, mobile home or manufactured home, North Dakota political subdivisions, repossession, non-resident military lease, total loss settlement or salvaged, and other miscellaneous categories. The form requires the inclusion of relevant information such as vehicle identification number, title number, year, make, and model. Additionally, there are various sections for lien holders, tax paid to state, dealer information, and exemptions. The buyer certifies the purchase price and makes an application for certificate of title, also certifying that the vehicle will be insured while operating on public streets and highways.
