Physically Disabled License Plate Application form 750-C (revised 01/2023) requires applicants to provide their name, address, city, state, zip code, daytime phone number, email address, current vehicle registration details including license plate number, year and make, Oklahoma title number, decal expiration date, placard number, and expiration date of the permit issued by Service Oklahoma. Applicants must pay a replacement fee and indicate whether they are applying for a single license plate or replacement decals to be displayed on the front and rear of the same vehicle.
