Unobtainable Ownership Documentation

This document is an unobtainable ownership documentation affidavit for a salvage certificate of title application by an insurance company. The form, which must be filled out and signed under penalty of perjury, requires the insurance company's name, FEIN, address, daytime phone number, email address, and the vehicle's year, make, model, VIN, Oklahoma title number, insured owner's name. The document also includes information about the total loss settlement, unsuccessful attempts to secure proper titling documentation from the insured owner, written attempt dates, and notarized signature of an authorized insurance company representative. The form is utilized following the provisions of 47 O.S. § 1105(P), which states that if an insurance company makes a total loss settlement on a vehicle and cannot obtain the properly endorsed certificate of ownership within thirty days, they may apply for a salvage title. Additionally, insurance companies are required to notify both the record owner and Service Oklahoma in writing of any damage percentage determination exceeding 60% of the market value or paying a claim for a flood-damaged vehicle.
