[Form 791-1A Revised 01/2023] This document is a Commercial/Utility Trailer Dealer License and Plate Application form. It requires the applicant to provide their business name, owner's name, FEIN or SSN, mailing address, trailer dealer number, person to contact for information, lot location address, date of application, number of temporary tags, expiration date of temporary tags, current issue, plates ordered, list of lost/stolen dealer plates, and missing decals. The form provides instructions on original application, renewal of current plates, replacement plates or decals, damaged plates, penalty fees, agreement, and payment details. The applicant is required to make checks payable to Service Oklahoma, return damaged plates, advise each purchaser about title requirements and taxes due the state, keep records, and make all reports required by Service Oklahoma.
