Used by an organization wishing to have a new special plate authorized for their membership
The document is an application for a new safety or service organization to receive special license plates from the Vermont Department of Motor Vehicles. The applicant must provide the name, mailing address, number of Vermont members, federal Employer ID or Taxpayer ID, nonprofit status under Section 501(c)(3) or (10), and registration as a nonprofit corporation with the Secretary of State. The organization's name and emblem must not be offensive, confusing, promote products, brands, or services, or infringe on proprietary rights. After approval, the organization pays a deposit of $2,200 to cover plate production costs and issuance fees for the first 100 members. Members may have special plates issued at no additional cost until the deposit is depleted, after which they pay a $17 fee. The organization may not charge its members any additional fees. Individuals must surrender their special plates upon membership termination, except for retired Vermont National Guard members who can renew or acquire them for a $10 fee.
